Below, we've answered some of the most frequently asked questions about neck pain, what may be contributing to it, how the nervous system may be involved, and what can be done to support better function.
You Shouldn't Have to Plan Your Life Around Your Neck Pain
Maybe you've learned which way you can turn your head without it hurting. You know exactly how to position your pillow. You avoid certain workouts. You shift your whole body to check your blind spot while driving.
Over time, those little accommodations can start to feel normal.
But living around your neck pain isn't the same as feeling well.

Constant neck tightness doesn't necessarily mean you simply need to stretch more.
Muscles respond to information coming from the nervous system. When your body perceives stress, instability, injury, or increased physical demand, muscles may tighten or guard as a protective response.
Long hours at a computer, repetitive movements, previous injuries, poor sleep, and chronic stress can all contribute to persistent tension.
That's why we like to look beyond the tight muscle itself and ask why your body may feel the need to keep that area tight in the first place.

Yes. Stress can show up physically in the body, and the neck and shoulders are common places to feel it.
When you're under stress, your nervous system can shift into a heightened state of alert. As part of that response, muscles may tense or brace, particularly around the neck, shoulders, and upper back. In the short term, that's a normal protective response. But when stress becomes ongoing, your body may have a harder time fully relaxing out of that state.
Over time, you may notice persistent neck and shoulder tension, stiffness, headaches, jaw clenching, or even the feeling that your shoulders are constantly creeping toward your ears.
This is one reason we look beyond the painful area itself. Sometimes the question isn't only "Why does my neck hurt?" but also "Why is my nervous system telling these muscles to stay so tense?"

Spending long periods looking down at a phone, tablet, or laptop can place additional demands on the muscles and joints of the neck.
The issue isn't that looking down is inherently harmful. Your neck is designed to move in many directions.
The bigger concern is often how long you stay in one position and how frequently you repeat it.
Taking movement breaks, changing positions throughout the day, improving workstation setup, and addressing underlying movement or nervous system dysfunction may all be helpful.

At the same time, headaches can have many different causes, including migraine, concussion, medication effects, illness, and other neurological or medical conditions.

In some cases, neck dysfunction or injury may contribute to feelings of dizziness or unsteadiness.
Your neck contains sensory receptors that continuously tell your brain about the position and movement of your head. Your brain combines this information with signals from your eyes and inner ear to help maintain orientation and balance.
If those signals aren't coordinating efficiently, particularly following an injury such as whiplash, some people may experience dizziness or a feeling of being "off."
Dizziness has many potential causes, so it shouldn't automatically be assumed to come from the neck.

Yes.
Sometimes a previous injury becomes relevant months or even years later.
After an injury, the body may compensate by changing movement patterns or increasing muscle guarding. Even after the initial pain improves, those patterns may persist.
Previous auto accidents, sports injuries, falls, and concussions are all important parts of your health history when evaluating chronic neck pain.

Yes.
The forces involved in a concussion can also affect the cervical spine and surrounding muscles and tissues.
This is one reason concussion symptoms and neck injury symptoms can sometimes overlap. Headaches, dizziness, neck pain, visual discomfort, and difficulty concentrating may occur following either injury.

It can.
The discs between the vertebrae act as cushions and help allow movement throughout the spine. A disc can bulge or herniate and, depending on its location and severity, may irritate nearby structures or nerves.
Symptoms can include:
Neck pain
Shoulder or arm pain
Numbness
Tingling
Weakness
Burning or electric sensations
Interestingly, imaging findings don't always perfectly match symptoms. Some people have disc changes without significant pain, while others experience considerable symptoms.
That's why we consider imaging alongside your symptoms, health history, neurological findings, and physical examination rather than looking at an MRI or X-ray in isolation.

Nerves exiting the cervical spine travel into the shoulders, arms, and hands.
When one of these nerves becomes irritated or compressed, symptoms may extend beyond the neck and include pain, numbness, tingling, burning, or weakness in the shoulder, arm, or hand.
Muscles and joints in the neck can also refer pain into nearby areas.
Because radiating symptoms can have several causes, a neurological examination can help determine which structures may be involved.

Numbness, tingling, weakness, or changes in sensation may indicate involvement of a nerve or another neurological structure.
That doesn't automatically mean something serious is occurring, but these symptoms provide important information and shouldn't simply be ignored.

The decision depends on your age, health history, symptoms, previous injuries, examination findings, and other clinical factors.
We have digital X-ray capabilities in our office for adult patients and use imaging when clinically appropriate.
If another form of imaging, such as MRI or CT, may be necessary, we'll recommend that you follow up with the appropriate medical provider.
